A Functional Capacity Assessment (FCA) aims to provide us with a holistic picture of you and your daily life, and to particularly understand what areas need to be supported and accommodated to help you can achieve quality of life, personal goals, and aspirations. The associated FCA report can be used to provide evidence to the NDIA for a range of reasons - to request access to the NDIS and create a NDIS plan, to change your existing NDIS plan to incorporate home and living supports, and to prepare for a NDIS Plan Reassessment. A Housing Assessment provides evidence about the specific home and living supports you need to have quality of life. These supports can include Short Term Accommodation (STA) or respite, Specialist Disability Accommodation (SDA), Supported Independent Living (SIL), and Individualised Living Options (ILO).

A Housing Plan provides evidence about your eligibility for SDA, SIL or ILO. It is also a way of communicating your preferences to the NDIA about your preferred support model, building location, and building type.

We can complete a Building Audit or Home Assessment to identify access and usability issues with your home. Our findings may lead us to recommending supports such as minor home modifications, Complex Home Modifications (CHM) or alternative housing options.

Our occupational therapists have successfully completed nationally recognised training modules CPPACC4020A and CPPACC5016A to be able to complete CHM assessments in line with the interim guidelines released by the National Disability Agency (NDIA). We can specify what you need to building service providers through an associated Scope of Works Report.

We are Assistive Technology (AT) assessors who can help you to find appropriate AT (or equipment) to improve your daily living. AT includes mobility aids, daily living aids, vehicle modifications, seating systems, fall prevention aids, pressure care aids, and Self-monitoring Analysis and Reporting Technology (SMART) technology. The NDIA requests AT Assessments and associated reports from qualified allied health practitioners to make sure that AT you are prescribed is economical and not going to put you at risk.
