Our practitioners are registered to provide Medicare M3 – Allied Health Services for Chronic Disease Management
If you have a chronic condition, you may be entitled to 5 occupational therapy sessions per calendar year using Medicare funding. A chronic condition is one that typically lasts more than 6 months.
To be eligible for the Chronic Disease Management Program, you must be referred by a GP or medical practitioner, and you must have either a GP Management Plan, Team Care Arrangements, or a multidisciplinary care plan. Medical practitioners must use the Department of Health and Aged Care referral form or our Protice Health Medicare M3 Referral Form form that contains the same components.
Out-of-pocket expenses per hourly session is $135.99, which is our gross rate of $193.99 (GST-free) less Medicare rebate of $58.00.
