NDIS Assessments & Reports

Our assessments and reports are completed with your unique background in mind, and are aligned with best practice frameworks and guidelines - evidence-based research, the NDIS Act (2013), The National Disability Insurance Scheme (NDIS) Independent Assessment Framework (2020), and the specific types of disability evidence the National Disability Insurance Agency (NDIA) prefers.

  • A FCA incorporates findings from informal observations and a range of evidence-based standardised assessments. We select the most appropriate assessment tools based on your medical and psychosocial backgrounds, with the aim of understanding the areas that need to be supported and accommodated to help you can achieve quality of life, personal goals, and aspirations. The associated FCA report can be used to provide evidence to the NDIA for a range of reasons - to request access to the NDIS, to request home and living supports, and to prepare for a NDIS Plan Reassessment. Contact us if you need a NDIS FCA report.

  • A Housing Assessment provides evidence about what you need to be able to live independently and safely. It is usually an assessment that will be completed in conjunction with your FCA.

A Housing Assessment can be used to support your request to the National Disability Agency (NDIA) for appropriate housing - Short Term Accommodation (STA), Individualised Living Options (ILO), Supported Independent Living (SIL), or Specialised Disability Accommodation (SDA).

  • We can help you change your built environment to make your home easier to access and use. A Home Modifications Assessment provides evidence about access and usability issues in your existing home.

An Home Modifications Assessment can be used to support your request to the National Disability Agency (NDIA) for minor home modifications or Complex Home Modifications (CHM).

Occupational therapists who work for Protice have successfully completed nationally recognised training modules CPPACC4020A and CPPACC5016A to be able to complete CHM assessments in line with the interim guidelines released by the National Disability Agency (NDIA).

  • A General Assistive Technology (AT) Assessment provides evidence about which item of AT best suits your needs and why it is needed for your everyday life. We can also help you to source and integrate AT into your life.

Our assessments are aligned with the NDIS Complexity Classification Guide.

  • We can start the process of changing your existing NDIS plan, so you can access the reasonable and necessary supports you need to achieve your individual goals.

Ongoing Occupational Therapy

Our occupational therapy programs typically focus on building your capacity to complete everyday activities by improving your physical, cognitive, sensory-perception, and psychosocial function. Activities of daily living include eating, grooming, bathing, dressing, toileting, and functional mobility. Instrumental activities of daily living can include use of telecommunication devices, laundry, shopping, transportation, medication management, housekeeping, and financial management. The frequency and duration of your occupational therapy program will depend on your individual goals.
